I agree to complete the training pathway that I have chosen. If I am unable to complete my training pathway, I agree to let LifeKeepers know (either verbally or in writing) as early as possible; if I have chosen an in-person training, I agree to let LifeKeepers know at least one week before the training so that my place can be offered to another participant.

I agree to complete all required assessments and evaluations for the LifeKeepers training pathway that I have chosen (pre- and post-training). If I do not complete the required assessments and evaluations, I might not be eligible to receive LifeKeepers certification.

I agree to be added to the LifeKeepers subscription database and to receive newsletters, and, from time to time, additional material related to the LifeKeepers programme.

I agree not to copy and share information from the LifeKeepers programme (such images, videos, or text from the workbook or e-learning environment) without first getting written permission from the LifeKeepers team and Te Whatu Hauora.

I agree to respect the privacy of other participants and will not share any personal details or personal information shared by participants as part of my chosen LifeKeepers training pathway.

I am aware that LifeKeepers is sponsored by Te Whatu Ora and therefore certain information will be used confidentially for reporting to Te Whatu Ora and for quality assurance.

Please note that if you fail to complete or attend your training pathway, without notifying us, you might not be eligible to enrol in LifeKeepers at a later date.

I am aware that LifeKeepers is only available to participants aged 18 years and over who are New Zealand Citizens/Residents and can confirm that I meet these requirements.
